



New patients are asked to arrive at least 10-15 minutes early so that they may fill out a New Patient Information form and other required paper work. In order to best complete the required paper work, patients should be familiar with the contact information of their employer, other parents or guardians, and emergency contacts. Knowledge of past medical history is also important.

Follow up patients, if applicable, may be required to bring test films such as MRI, CT scan for review.
Up to date insurance cards must ALWAYS be presented to the front desk at every appointment. Co-pay is due at the time of each visit.
We appreciate your foreseeing the need to refill your prescriptions. Notify your pharmacist early in the day and have our office contacted with the necessary request. This can only be accomplished during our normal working hours and preferably before 3:00 P.M. Depending upon your prescription, a physician visit may be necessary in order for you to obtain a renewal.
